A handover card that says what actually happened

Leave the next carer a useful record, with room for an unfinished job.

Fill it in after the task

A good handover separates completed care from the next job. Keep one current card where the next carer will look. Use the pet’s name if you have more than one. Cross out an old instruction when it changes so two versions do not compete.

Keep medical instructions in the existing professional care plan. The card can say where that plan is stored; it should not become a second, abbreviated set of doses. Keep personal contact and access information private.

Fictional example

Completed: evening meal served at 6:15 by Carer A, following the existing food instructions. Next: Carer B takes the agreed evening outing. Supplies: waste bags moved to the hook beside the lead. Unfinished: refill the bag holder before leaving. Carer B confirms the note was read.

Avoid “all sorted.” It saves a few words but makes the next person reconstruct what you meant. If nobody has acknowledged the handover, contact the person directly instead of assuming the note was seen.
